Working with GPs to get the best community care

 Immunisation reminders  Mental health initiatives

After hours services

CNWQML has finalised contracts with eight of the 14 General Practices in the region to deliver after hours services. Service Agreements were designed in consultation with a number of General Practices within the CNWQML region to assist maintaining vital After Hours Service. Details of after hours GP services are available here.

A Reminder System is in place which sends a ‘Monthly Birthday Reminder Card’ on behalf of general practices. The objective is to assist general practices to maintain/increase childhood immunisation rates, which is currently running at higher than 90% in the region.

After Hours Mental Health Orientation Package

The package has been the result of a  collaboration to ensure that people with a mental illness receive the best care possible when presenting to a health facility after hours. Discussion about the delivery of services after hours identified the need for simple, packaged information for health professionals to support proper referrals, assessment and follow up. The new resource is available for local General Practitioners, nursing/allied health professionals, medical students and other medical staff. The project was the result of collaboration between the Central and North West Queensland Medicare Local (CNWQML) and the Mental Health Team of the Central West Hospital and Health Service (CWHHS). The project included input from local general practitioners and the RFDS.
